How to become a patient of RL-Corpus s.r.o

RL-Corpus is an ambulant center.
We offer intensive therapy block, which means 3-5 days of therapy. This includes diagnosing according to Vojta, setting the therapy plan and teaching the parent, how to practise
the therapy at home.
It is necessary to realize, that the main part of the therapy according to Vojta lies in the home therapy, not in the therapy that the physiotherapist does with the child in our center.

Therapy enrolment/registration:
1.    download the application form
2.    fill in the application form according to the instructions on the bottom part of the form.

For an easier and faster communication, please:
•    Write always from the same email address
•    In the „subject“ of the email write the name of your child
•    Do not intentionally repeat the emails
•    Please follow the rules stated to help us make the communication easier and faster

Another important information:
•    It is in competition of Mgr. Michaela Kubisová to assign a new patient to a particular therapist. We ask you to be patient, as this process is not always easy. We always try to work on every application form delivered as soon as possible. We do have priorities in assigning a patient to our center, which come mainly from the age of the child. We always try to include every child into the therapy.
    
In case that you want to combine our therapy with a therapy in different center, please inform us right away, so we can find the best solution to fit your child.

The price of a 60 minutes therapy session is 1380 CZK.
The therapies are often shorter, with a corresponding price
(45 minutes for 1035 CZK, 30 minutes for 690 CZK, 15 minutes for 345 CZK).
The services of a translater we can book for you, if it is necessary. The price for 60 minutes is 450 CZK, tax included.
 
We advise you to bring a camera, so in the last therapy you can take photos or videos of the positions, so they can help you with home therapy.

Cancelation
We accept cancelations due to health reasons. This should be done 3 weeks before the therapy starts, if possible.
Please note, that there are patients waiting for the dates to come with their child and when the therapy is cancelled too late, or not at all (which unfortunately happens), we do not have a chance to give your date to another child.
